RN presented to patient’s room at 0400 to assess patient’s vitals. Patient was found sitting on the ground to the right side of the bed, with his call light remaining on his bed clipped to his sheets. Bed alarms were not activated. Patient stated that he had to go to the bathroom and was trying to walk there. He was disoriented to time and President. Patient did not recall hitting his head. A hoyer lift was utilized to transfer patient back to bed and Neuro screen was repeated. Noted worsening left-sided arm and leg weakness and confusion. Paged Hospitalist who ordered a repeat head CT. Will continue to monitor.
