RN was called to patient’s room by occupational therapist during their evaluation around 1300 on 10/3/24. Occupational therapist was concerned that patient was requiring heavier assistance with mobility than he was previously in day. He was requiring moderate assistance to transfer/ambulate, and was having significant difficulty ambulating to bathroom. RN noted acute change with neuro screen, specifically with inability to grip writer’s hand and visually scan to the left. Code stroke was called and SWAT team arrived, patient taken for head CT. Will continue to monitor.
